Tetrahydrozoline
Tetrahydrozoline is a direct-acting sympathomimetic alpha-adrenergic agonist that causes vasoconstriction of conjunctival and nasal blood vessels by stimulating α1 and α2 receptors, producing rapid reduction of redness and congestion. It is available over-the-counter as ophthalmic drops for eye redness and as nasal drops/sprays for temporary nasal decongestion. Prolonged use can lead to rebound congestion (rhinitis medicamentosa) due to down-regulation of adrenergic receptors.
