The combination of empagliflozin and hydrochlorothiazide can cause additive volume depletion and hypotension, and may also increase the risk of genital mycotic infections through shared glucosuria.
Mechanismus
Both agents reduce plasma volume through independent renal mechanisms (SGLT2 inhibition vs. NCC inhibition); combined natriuresis and diuresis heighten orthostatic hypotension risk. Persistent glucosuria from empagliflozin combined with potential hyperglycaemia from thiazide-induced insulin resistance may worsen glycaemic control.
Klinische Bedeutung
Real-world observational data indicate a higher incidence of volume depletion events when these agents are combined, particularly during hot weather or intercurrent illness.
Maßnahmen
Monitor blood pressure and hydration status; counsel patients on maintaining adequate fluid intake; consider hydrochlorothiazide dose reduction if volume depletion occurs; monitor fasting glucose and HbA1c.
