Same Drug Class

Lansoprazole vs Pantoprazole

## Overview

Lansoprazole and pantoprazole are both effective PPIs, but pantoprazole's minimal CYP2C19 inhibitory activity gives it a clear advantage in patients on antiplatelet or other CYP2C19-sensitive drugs.

## Mechanism of Action

Both irreversibly inhibit H+/K+-ATPase. Both require enteric coating or delayed-release formulation to reach the duodenum intact.

## Pharmacokinetics

Lansoprazole: CYP2C19 + CYP3A4 metabolism. Pantoprazole: CYP2C19 + CYP3A4, but pantoprazole inhibits CYP2C19 much less potently than lansoprazole. Both have ~1–1.5 h half-lives with IV formulations available.

## Clinical Evidence

Equivalent efficacy for GERD, peptic ulcer, and H. pylori eradication. Pantoprazole's IV formulation is extensively used in hospital settings. Lansoprazole's SoluTab (ODT) is useful for nasogastric tube administration.

## Key Takeaways

- Pantoprazole: fewer drug interactions, preferred with clopidogrel
- Lansoprazole: SoluTab ODT formulation unique advantage
- Both available as IV; equivalent for standard indications

Veredicto

Pantoprazole is preferred with clopidogrel or other CYP2C19-sensitive drugs. Lansoprazole SoluTab is preferred for patients with swallowing difficulties or nasogastric tube administration. For standard outpatient GERD, both are equivalent.
