Sertraline vs Escitalopram

## Overview

Sertraline and escitalopram are the two SSRIs most commonly ranked as first-line agents in contemporary treatment guidelines and network meta-analyses. Both have excellent tolerability, minimal drug interactions, and proven efficacy across multiple indications.

## Mechanism of Action

Both inhibit SERT. Escitalopram additionally binds an allosteric site on SERT that may amplify its primary inhibitory effect, potentially explaining marginally faster onset of action in some studies.

## Pharmacokinetics

Sertraline: half-life 26 h, multiple CYP enzymes, minimal CYP inhibition. Escitalopram: half-life 27–32 h, primarily CYP2C19, minimal CYP inhibition. Both are similar in PK terms.

## Clinical Evidence and Efficacy

The Cipriani 2018 network meta-analysis placed escitalopram #1 for efficacy + acceptability; sertraline ranked highly too (top 3–4). Head-to-head trials show escitalopram has slightly but statistically significantly better response rates in some studies. However, effect sizes are small and clinical significance of the difference is debated.

## Indications

Sertraline: MDD, OCD, panic disorder, PTSD, PMDD, social anxiety disorder (6 FDA indications). Escitalopram: MDD, GAD (2 FDA indications, but widely used off-label for same spectrum). For PTSD specifically, sertraline is one of only two FDA-approved drugs (along with paroxetine).

## Safety

Both have mild QTc effects at high doses. Escitalopram carries the FDA dose limit of 20 mg/day. Neither has significant CYP interactions. Discontinuation syndrome is mild for both.
