Valsartan vs Candesartan

## Overview

Valsartan and candesartan are both widely used ARBs with strong HF evidence and similar pharmacological profiles. Candesartan has slightly higher AT1 receptor affinity and stronger HFrEF-specific trial evidence, while valsartan is better studied post-MI.

## Mechanism of Action

Both block AT1 with high affinity. Candesartan's binding is insurmountable; valsartan's binding is surmountable but has high AT1 selectivity (at1:at2 ratio very high).

## Clinical Evidence

Val-HeFT (valsartan added to standard therapy in HFrEF) and VALIANT (valsartan vs. captopril post-MI HF). CHARM-Alternative and CHARM-Added (candesartan) also in HFrEF. In terms of the ACEi-intolerant patient, candesartan has direct evidence from CHARM-Alternative; valsartan from Val-HeFT (most patients were on ACEi).

## Key Takeaways

- Both ARBs with strong HF evidence; candesartan stronger in ACEi-intolerant HF
- Valsartan: better post-MI data (VALIANT); preferred post-MI LV dysfunction in ACEi-intolerant
- Candesartan: insurmountable AT1 binding, CHARM-Alternative for ACEi intolerant

Veredicto

Candesartan is the preferred ARB for ACEi-intolerant HFrEF patients based on CHARM-Alternative. Valsartan is preferred in post-MI LV dysfunction (VALIANT). Both are acceptable for hypertension.
