Doxycycline is reported to modestly inhibit CYP3A4, potentially increasing amlodipine plasma levels; the clinical consequence is minor given amlodipine's wide therapeutic index.
Mecanismo
Amlodipine undergoes extensive CYP3A4-mediated hepatic metabolism; weak CYP3A4 inhibition by doxycycline at therapeutic concentrations may marginally reduce amlodipine clearance.
Relevancia clínica
No clinical case reports of significant amlodipine adverse effects attributable to doxycycline exist; the interaction is predicted based on in-vitro and limited in-vivo data.
Manejo clínico
No dose adjustment required; routine blood pressure monitoring during antibiotic therapy is sufficient.
