Description
Pantoprazole has minimal CYP2C19 inhibitory activity compared with omeprazole, making it the preferred PPI choice in patients on clopidogrel.
Mécanisme
Pantoprazole does not significantly inhibit CYP2C19, so clopidogrel bioactivation is largely preserved compared with omeprazole or esomeprazole.
Signification clinique
Observational data suggest no significant increase in cardiovascular events with pantoprazole + clopidogrel; this interaction is less clinically significant than the omeprazole-clopidogrel combination.
Prise en charge
Pantoprazole is the preferred PPI for gastroprotection in patients on clopidogrel; routine monitoring is not required beyond standard care.
