Description
Combining rivaroxaban with clopidogrel substantially increases bleeding risk through simultaneous inhibition of factor Xa and platelet P2Y12 receptors.
Mécanisme
Rivaroxaban inhibits factor Xa, preventing thrombin generation and fibrin clot formation; clopidogrel irreversibly blocks ADP-mediated platelet activation. The combined antithrombotic burden is greater than either alone.
Signification clinique
Clinical trial data (PIONEER AF-PCI) demonstrate significantly higher bleeding rates with this combination, particularly GI hemorrhage.
Prise en charge
Minimize duration of dual therapy to the lowest risk-stratified period; add gastroprotection; reassess at each visit whether both agents are still needed.
