Description
Combining empagliflozin with an ACE inhibitor such as ramipril can cause additive blood pressure reduction and volume depletion, increasing the risk of hypotension and acute kidney injury.
Mécanisme
Empagliflozin lowers blood pressure through osmotic diuresis and natriuresis; ramipril reduces angiotensin II-mediated vasoconstriction and aldosterone production. The combination produces additive antihypertensive and volume-depleting effects.
Signification clinique
EMPA-REG OUTCOME trial, in which the majority of patients were on ACE inhibitors, showed increased volume depletion-related adverse events; particularly relevant in elderly or volume-depleted patients.
Prise en charge
Assess volume status before initiating empagliflozin; monitor blood pressure and renal function within 2 weeks; counsel patients on adequate fluid intake and reporting symptoms of dizziness or syncope.
