Combined use of carvedilol and lisinopril in heart failure is guideline-recommended but carries additive hypotensive risk, particularly with initiation or dose uptitration.

Carvedilol provides alpha-1 and non-selective beta blockade; lisinopril reduces angiotensin II–mediated vasoconstriction. Additive vasodilation and negative inotropy can cause hypotension.

The combination is standard-of-care in HFrEF; benefits include mortality reduction; the primary risk is hypotension, dizziness, and fatigue, especially early in therapy.
प्रबंधन
Initiate at lowest doses; uptitrate each agent separately; monitor blood pressure and renal function throughout titration.
