Treprostinil
Treprostinil is a synthetic stable analog of prostacyclin (PGI2) that binds to prostacyclin receptors (IP receptors) on pulmonary vascular smooth muscle cells, activating adenylyl cyclase, increasing intracellular cAMP, and causing potent pulmonary and systemic vasodilation while also inhibiting platelet aggregation and vascular smooth muscle cell proliferation. It is approved for pulmonary arterial hypertension (PAH) via multiple routes (subcutaneous, intravenous, inhaled, oral) and is under investigation for COPD-associated pulmonary hypertension. Its chemical stability distinguishes it from natural prostacyclin, allowing storage at room temperature.
