Thallous chloride Tl 201 is a radioactive potassium analog that is taken up by viable myocardial cells via the Na+/K+-ATPase pump proportional to regional myocardial blood flow and cellular viability, historically used as the primary myocardial perfusion imaging agent for detection of coronary artery disease by SPECT. The 201Tl emits mercury x-rays and gamma photons (167 keV) detectable by gamma camera. Its redistribution property over time allows distinction of ischemic (reversible perfusion defect) from infarcted (fixed defect) myocardium in a single injection rest-redistribution protocol.
