First approval for any HIV treatment went to zidovudine, still widely known as AZT, and the drug inhibits reverse transcriptase, the enzyme the virus requires to copy its RNA genome into DNA. Blocking that conversion halts replication within infected cells. Contemporary use places it inside combination antiretroviral regimens rather than alone, and it retains a specific role in preventing mother-to-child transmission, given to pregnant women with HIV and to their newborn infants. Bone marrow suppression with resulting anaemia is the principal toxicity and the reason haematological monitoring accompanies treatment.
