Costimulation is the second signal a T cell requires before it can be fully activated, and abatacept blocks it. Depriving T cells of that signal dampens the overactive immune response driving inflammation and tissue damage in autoimmune disease, an approach upstream of the cytokine-directed biologics. Rheumatoid arthritis, psoriatic arthritis and juvenile idiopathic arthritis are the conditions treated, with administration by intravenous infusion or weekly subcutaneous injection. Because the drug suppresses T-cell activation, live vaccines are avoided during treatment and patients are monitored for signs of infection. The product is a protein therapeutic whose development has reached the final clinical phase.
