Sotatercept is a fusion protein consisting of the extracellular domain of activin receptor type IIA (ActRIIA) fused to the Fc region of human IgG1, acting as a ligand trap for TGF-beta superfamily members including activin A, to treat pulmonary arterial hypertension. By sequestering hyperactivated TGF-beta/activin signaling, it counteracts pulmonary vascular smooth muscle cell proliferation and vascular remodeling in PAH. Clinical trials demonstrated significant improvements in exercise capacity and hemodynamics in PAH patients.
