Androgen synthesis continues in the adrenal glands and within tumour tissue even after testicular suppression, and abiraterone acetate addresses that residual production by inhibiting CYP17, an enzyme required at an early step of the pathway. Lowering androgen concentrations throughout the body makes the drug effective in advanced prostate cancer that progresses despite standard hormone therapy. Blocking CYP17 also diverts steroid synthesis toward mineralocorticoids, which is why a corticosteroid such as prednisone accompanies treatment to offset the resulting hormonal effects. Blood pressure and hepatic function are monitored during therapy, both reflecting predictable consequences of altered steroid synthesis. Abiraterone acetate is a prodrug hydrolysed to abiraterone, a selective and irreversible inhibitor of CYP17A1 (17-alpha-hydroxylase / C17,20-lyase). Blocking this single enzyme halts androgen synthesis in the testes, the adrenal glands and within prostate tumour tissue itself. Because CYP17A1 blockade diverts precursors into the mineralocorticoid pathway, it is given together with a corticosteroid to suppress the resulting ACTH drive.
