Dopamine D2 receptors in the mesolimbic pathway are the principal target of acetophenazine, a phenothiazine antipsychotic used for schizophrenia and related severe psychiatric illness. Reducing excessive dopaminergic transmission along that pathway diminishes hallucinations, delusions and disorganised thinking. Antagonism at serotonin 5-HT2A receptors accompanies the dopaminergic action in many agents of this family and is thought to influence negative symptoms and the liability to movement disorder. Extrapyramidal effects, including tremor and involuntary movement, remain the characteristic burden of the class, and newer antipsychotics with better tolerability have largely displaced it from current practice.
