Aminohippuric Acid
Measurement rather than treatment is the purpose of para-aminohippuric acid, a diagnostic agent used to quantify renal plasma flow and tubular secretory capacity. The kidney extracts it from plasma almost completely in a single pass, combining glomerular filtration with active tubular secretion, so its clearance approximates renal plasma flow more closely than that of any endogenous marker. The same near-complete extraction makes the substance useful for probing the secretory capacity of the proximal tubule when that function is in question. It is a small molecule with the formula C9H10N2O3 and a molecular weight close to 194.2 g/mol. The agent is approved and has completed the final phase of clinical development.
Para-aminohippuric acid (PAH) is used as a diagnostic tool to assess kidney function, specifically renal plasma flow and tubular secretory capacity. The kidney almost completely removes it from the blood in a single pass, making the clearance rate a reliable measure of renal blood flow.

Para-aminohippurate is filtered at the glomerulus and then almost completely secreted by the organic anion transporters of the proximal tubule, so a single pass through the kidney clears it from plasma. Its clearance therefore approximates effective renal plasma flow, and at high plasma concentrations, where the secretory transporters saturate, the transport maximum measures proximal tubular secretory capacity.
