Technetium Tc 99m tilmanocept is a radiolabeled mannose receptor (CD206)-targeted sentinel lymph node mapping agent consisting of a dextran backbone conjugated with multiple mannose residues and DTPA chelators, providing highly specific and rapid binding to macrophages and dendritic cells in sentinel lymph nodes. It is approved for intraoperative sentinel lymph node identification in breast cancer, melanoma, and head and neck cancer to guide surgical staging. Its receptor-mediated retention in sentinel nodes is superior to passive colloidal accumulation, reducing the risk of false-negative results.
