Doxycycline may slightly enhance the glucose-lowering effect of metformin by improving insulin sensitivity through anti-inflammatory mechanisms, occasionally causing mild hypoglycaemia.
Механизм
Tetracyclines have been shown to reduce inflammatory cytokine-mediated insulin resistance and may independently improve glucose metabolism; combined with metformin's AMPK activation, a small additive pharmacodynamic effect is possible.

The interaction is generally not clinically significant; limited case reports describe mild hypoglycaemia. Large population studies have not confirmed a consistent effect.

No specific precautions required for most patients; monitor blood glucose in patients with tight glycaemic control if prolonged doxycycline courses are prescribed.
