Spesolimab is a humanized monoclonal antibody that blocks the interleukin-36 receptor (IL-36R), preventing signaling by the pro-inflammatory cytokines IL-36α, IL-36β, and IL-36γ, which drive keratinocyte-mediated inflammation in generalized pustular psoriasis (GPP). By inhibiting IL-36 receptor signaling, it rapidly suppresses the acute inflammatory cascade causing sterile pustule formation and systemic inflammation in GPP flares. It is approved for the treatment of GPP in adults and has orphan drug status for this rare condition.
