Zenocutuzumab (MCLA-128) is a bispecific antibody targeting HER2 and HER3 that blocks the binding of neuregulin-1 (NRG1) to HER3, thereby inhibiting HER2-HER3 heterodimerization and downstream PI3K/AKT and MAPK signaling driven by NRG1 fusion oncoproteins. It is under investigation for NRG1 gene fusion-positive cancers, including non-small cell lung cancer, pancreatic cancer, and breast cancer, where NRG1 fusions drive constitutive HER2-HER3 pathway activation. Its mechanism is particularly relevant for the small subset of cancers harboring NRG1 rearrangements.
