Repeated acetaminophen use may modestly lower lamotrigine plasma levels through induction of UGT glucuronidation pathways, though the clinical impact is generally small.
Mekanizma
Both acetaminophen and lamotrigine are metabolised via UGT1A4 glucuronidation; competition for this pathway or mild induction by acetaminophen could reduce lamotrigine bioavailability.
Klinik Önemi
Case reports suggest seizure breakthrough has occurred in a small number of patients with epilepsy during regular high-dose acetaminophen use; data are limited.
Yönetim
Monitor for any change in seizure frequency when adding or withdrawing regular acetaminophen in patients stabilised on lamotrigine; no routine dose adjustment is required.
