Technetium Tc 99m tetrofosmin is a lipophilic diphosphine radiopharmaceutical that accumulates in viable myocardium proportional to perfusion and mitochondrial membrane potential, used for myocardial perfusion SPECT imaging to evaluate coronary artery disease, assess myocardial viability, and risk-stratify patients with cardiovascular disease and hypertension. Its favorable dosimetry and imaging properties, with rapid blood clearance and low liver uptake, make it well-suited for same-day rest-stress protocols. It is one of the two most widely used myocardial perfusion agents alongside sestamibi.
