Spectinomycin is an aminocyclitol antibiotic with a mechanism similar to aminoglycosides, inhibiting bacterial protein synthesis by binding to the 30S ribosomal subunit and interfering with mRNA translocation, used primarily as an alternative treatment for uncomplicated gonorrhea in patients who cannot tolerate cephalosporins or fluoroquinolones. Unlike aminoglycosides, it is not nephrotoxic or ototoxic at therapeutic doses. Its investigational use in stroke is based on potential neuroprotective properties unrelated to its antibiotic activity.
