Tilmanocept is a radiolabeled receptor-targeted sentinel lymph node mapping agent consisting of a dextran scaffold conjugated with mannose residues (which bind CD206/mannose receptors on macrophages in sentinel lymph nodes) and DTPA chelators for technetium-99m radiolabeling. It is approved for intraoperative sentinel lymph node identification in solid tumors including breast cancer and melanoma to guide surgical staging and lymph node dissection decisions. Its receptor-targeted mechanism provides superior node retention compared to passive particle accumulation, reducing false-negative sentinel node biopsies.
