Duloxetine
Raising the availability of two neurotransmitters at once, duloxetine inhibits the reuptake of both serotonin and norepinephrine, placing it in the serotonin-norepinephrine reuptake inhibitor class. The dual action supports mood while also engaging pain-modulating pathways in the nervous system, which is why the drug treats not only depression and anxiety but also chronic nerve pain and fibromyalgia. A small molecule (C18H19NOS) with a half-life of about 12 hours, it is generally dosed once or twice daily. Its effect on descending pain pathways distinguishes it from antidepressants that act on serotonin alone and underlies its use in painful physical conditions. Duloxetine is an approved medicine bridging psychiatric and pain-related indications.
A serotonin-norepinephrine reuptake inhibitor (SNRI) used to treat depression, anxiety, nerve pain, and fibromyalgia. It works by increasing the availability of serotonin and norepinephrine in the brain.

Duloxetine, a serotonin-norepinephrine reuptake inhibitor, combined with tramadol markedly elevates the risk of serotonin syndrome.
Fluoxetine is a potent CYP2D6 inhibitor that substantially increases duloxetine plasma levels, while both agents also increase serotonin syndrome risk.
Combining duloxetine and sertraline, two serotonergic antidepressants, markedly increases the risk of serotonin syndrome without added efficacy.
Duloxetine and pregabalin are commonly co-prescribed for neuropathic pain with complementary mechanisms; additive CNS effects including dizziness, somnolence, and cognitive impairment can occur.
Duloxetine and sumatriptan co-administration carries a serotonin syndrome risk due to complementary mechanisms of serotonergic stimulation.
Carbamazepine substantially reduces duloxetine plasma levels through CYP1A2 and CYP3A4 induction, risking loss of antidepressant efficacy.
