Vericiguat is an oral soluble guanylate cyclase (sGC) stimulator that directly activates sGC independent of nitric oxide (NO) availability and sensitizes sGC to low levels of NO, increasing cyclic GMP (cGMP) production in vascular smooth muscle and cardiomyocytes. It is approved for reducing the risk of cardiovascular death and heart failure hospitalization in adults with symptomatic chronic heart failure with reduced ejection fraction (HFrEF) following a worsening heart failure event. Increased cGMP mediates vasodilation, reduced cardiac afterload, and cardioprotective signaling.
