Description

NSAIDs combined with apixaban significantly elevate the risk of GI and other bleeding through additive effects on hemostasis and gastric mucosal injury.

Mécanisme

Ibuprofen impairs platelet aggregation via COX-1 inhibition and causes direct gastric mucosal damage; apixaban provides anticoagulant background, transforming minor mucosal lesions into major hemorrhage.

Signification clinique

Observational studies confirm substantially higher GI bleeding rates; hospitalization and transfusion requirements increase markedly.

Prise en charge

Avoid concurrent use. Substitute acetaminophen for analgesia; if NSAID is necessary, use the lowest dose for the shortest time and add a PPI.
