Description
Carbamazepine dramatically reduces quetiapine plasma concentrations, potentially rendering antipsychotic treatment ineffective.
Mécanisme
Carbamazepine is a potent CYP3A4 inducer; quetiapine is primarily metabolized by CYP3A4, and co-administration reduces quetiapine AUC by approximately 87%.
Signification clinique
Sub-therapeutic quetiapine levels risk psychiatric relapse; published clinical studies confirm the 5-fold or greater reduction in quetiapine exposure.
Prise en charge
Avoid combination; if both are required, markedly increase quetiapine dose (5× or more) with close monitoring, or switch to an antipsychotic less affected by CYP3A4 induction.
