Osimertinib vs Erlotinib
## Overview
Osimertinib (Tagrisso) and erlotinib (Tarceva) are both EGFR tyrosine kinase inhibitors (TKIs) for EGFR-mutated NSCLC, but osimertinib is a third-generation, CNS-penetrant agent that irreversibly inhibits both EGFR-activating mutations (Exon 19 del, L858R) and the T790M resistance mutation. Erlotinib is a first-generation reversible EGFR TKI.
## Mechanism of Action
Both inhibit EGFR (ErbB1) tyrosine kinase activity, blocking downstream RAS/MAPK and PI3K/Akt signaling. Erlotinib competes reversibly with ATP at the kinase domain; osimertinib covalently binds Cys797 in the EGFR kinase domain (irreversible inhibition) and has greater selectivity for mutant EGFR over wild-type.
## Clinical Evidence
FLAURA trial (osimertinib vs erlotinib/gefitinib 1st-line in EGFR-mutated NSCLC): osimertinib demonstrated superior PFS (18.9 vs 10.2 months) and OS (38.6 vs 31.8 months). Osimertinib is now the standard first-line treatment for EGFR-mutated NSCLC.
Erlotinib was historically first-line but is now predominantly used when osimertinib is unavailable. Erlotinib also has FDA approval for pancreatic cancer (with gemcitabine) and for NSCLC maintenance therapy — neither of which are osimertinib indications.
## CNS Penetration
Osimertinib's unique advantage: superior CNS penetration, providing CNS efficacy against brain metastases (FLAURA and LAURA trials). Brain metastases are common in EGFR-mutated NSCLC. Erlotinib has minimal CNS penetration.
## T790M Resistance
Erlotinib and first-generation TKIs inevitably lead to T790M resistance mutation (~50–60% of cases). Osimertinib is effective against T790M. When progression occurs on osimertinib, C797S mutation or MET amplification are typical resistance mechanisms.
## Key Takeaways
- Osimertinib: 3rd generation, irreversible, T790M-active, CNS-penetrant, first-line standard
- Erlotinib: 1st generation, reversible, limited CNS, now primarily second-line or for pan-cancer unavailability
- Osimertinib is the current standard of care for first-line EGFR-mutated NSCLC
निष्कर्ष
Osimertinib is the first-line standard of care for EGFR-mutated NSCLC (Exon 19 del or L858R), having demonstrated superior OS in FLAURA. Erlotinib is now primarily used when osimertinib is unavailable, as a subsequent-line option, or for its unique pancreatic cancer indication.
