Tamsulosin reduces peripheral vascular resistance via alpha-1 adrenoceptor blockade; sildenafil inhibits PDE-5 in smooth muscle, increasing cGMP-mediated vasodilation. The pharmacodynamic combination causes additive blood pressure reduction, particularly during standing.
