Description
Sertraline and sumatriptan may interact to increase serotonergic tone, raising the theoretical risk of serotonin syndrome.
Mechanism
Sertraline inhibits serotonin reuptake (SERT); sumatriptan is a 5-HT1B/1D agonist, and combined serotonergic stimulation may be sufficient to produce serotonin toxicity.
Clinical Significance
Most clinical evidence suggests the absolute risk is low when triptans are used at recommended doses, but post-marketing cases are documented.
Management
Combination is commonly encountered; use the lowest effective triptan dose and monitor for serotonin syndrome symptoms.
