Vilanterol is a long-acting beta-2 adrenergic agonist (LABA) with ultra-long duration of action (>24 hours) that activates beta-2 receptors in bronchial smooth muscle, producing sustained bronchodilation through cAMP-mediated relaxation. It is used exclusively in combination with inhaled corticosteroids (fluticasone furoate) or as a dual bronchodilator (with umeclidinium) for maintenance treatment of asthma and chronic obstructive pulmonary disease (COPD). Its 24-hour duration enables once-daily dosing.
