Drug-induced damage to the heart, including cardiomyopathy, arrhythmias, QT prolongation, heart failure, and myocardial ischemia. hERG potassium channel inhibition is the most common mechanism for QT prolongation and Torsades de Pointes risk. ICH S7B guidelines mandate cardiac safety pharmacology assessment in preclinical development.
