Stiripentol
Stiripentol is a structurally unique antiepileptic agent that acts through multiple mechanisms including potentiation of GABA-A receptor function, inhibition of voltage-dependent sodium channels, and inhibition of cytochrome P450 enzymes (CYP2C19 and CYP3A4), used for the adjunctive treatment of seizures in Dravet syndrome (severe myoclonic epilepsy of infancy) in patients also receiving clobazam and valproate. By inhibiting the metabolism of clobazam to its active metabolite norclobazam, and by directly potentiating GABAergic inhibition, it provides synergistic anticonvulsant effects. It is an orphan drug specifically indicated for this pharmacogenomically defined syndrome.
