Tissue-selective behaviour is what makes tamoxifen a selective estrogen receptor modulator rather than a simple antagonist: it blocks estrogen receptors in breast tissue while exerting estrogen-like effects elsewhere, notably on the endometrium, which is the basis for gynaecological surveillance during treatment. In hormone receptor-positive breast cancer the antagonist action both treats established disease and reduces the risk of new disease, in men as well as women, with adjuvant courses conventionally extending over five to ten years.
