Tolcapone
This medication is used as an add-on treatment to levodopa/carbidopa therapy in Parkinson's disease patients who are experiencing fluctuations in symptom control ("wearing off" episodes). It works by blocking an enzyme called COMT, which breaks down levodopa, allowing more of it to reach the brain. Due to the risk of serious liver damage, it requires regular liver function monitoring and is only used when other options have not worked.
