Combining gabapentin and pregabalin, two alpha-2-delta ligands with similar mechanisms, produces additive CNS depression and dose-dependent adverse effects including respiratory depression, dizziness, and somnolence.

Both drugs bind to the alpha-2-delta subunit of voltage-gated calcium channels, reducing excitatory neurotransmitter release; additive binding produces supra-additive CNS depression without additional analgesic benefit.

Clinical guidelines generally advise against combining these agents; FDA has added respiratory depression warnings to gabapentinoid labels; co-prescription without indication adds risk without benefit.

Xử trí

Avoid concurrent use; if both are prescribed, use the lowest effective doses and monitor for respiratory depression, especially in opioid users or those with sleep apnoea.
