Trandolapril is a prodrug ACE inhibitor that is hydrolyzed in the liver to its active diacid form trandolaprilat, which inhibits angiotensin-converting enzyme, reducing angiotensin II production and aldosterone secretion, leading to vasodilation and decreased sodium retention. It is approved for hypertension, heart failure post-myocardial infarction, and left ventricular dysfunction after MI, and provides cardioprotective benefits beyond its antihypertensive effects.
