Tipiracil is a thymidine phosphorylase inhibitor that blocks the enzymatic degradation of trifluridine (TAS-102 combination partner) in the gut and blood, maintaining therapeutic trifluridine plasma concentrations. When combined with trifluridine, it enables sufficient systemic trifluridine exposure to incorporate into tumor cell DNA and inhibit thymidylate synthase, exerting cytotoxic effects. The combination is approved for refractory metastatic colorectal cancer and gastric cancer.
