Cardiovascular research, not sexual medicine, produced sildenafil, a selective inhibitor of phosphodiesterase type 5. Preventing the breakdown of cyclic GMP sustains the second messenger that relaxes vascular smooth muscle, so blood vessels dilate and flow increases. Two indications follow from the same mechanism applied to different vascular beds: erectile dysfunction, and pulmonary arterial hypertension where the aim is to lower pressure in the pulmonary arteries. Concurrent nitroglycerin is recorded as a major interaction, because nitrates and PDE5 inhibition act on the same cGMP pathway and together can precipitate severe hypotension. The compound has a half-life near four hours and the formula C22H30N6O4S, about 474.6 g/mol.
