Description
Carbamazepine may reduce levodopa efficacy through enzyme induction and pharmacodynamic antagonism, potentially worsening Parkinson's disease control.
Mechanism
Carbamazepine induces hepatic enzymes that metabolize levodopa's carbidopa component, potentially increasing peripheral levodopa breakdown; it may also block dopaminergic effects centrally.
Clinical Significance
Worsening motor function has been reported in Parkinson's patients starting carbamazepine; tremor and rigidity may increase.
Management
Monitor motor function closely; levodopa dose increase may be needed; consider alternative anticonvulsants (lamotrigine, gabapentin).
