Description

Gabapentin and valproic acid have a minor pharmacokinetic interaction; valproate may slightly increase gabapentin renal clearance.

Mechanism

Gabapentin is renally excreted unchanged; valproate may slightly alter renal tubular handling of gabapentin but the clinical effect is minor.

Clinical Significance

The interaction is generally not clinically significant; however, combined CNS depressant effects can cause additive sedation.

Management

Monitor for excessive sedation; dose adjustments are rarely required for the pharmacokinetic component of this interaction.
