Trabectedin
Trabectedin (ecteinascidin-743, ET-743) is a marine-derived tetrahydroisoquinoline alkaloid that binds to the minor groove of DNA, distorting the DNA helix, stalling RNA polymerase II, and triggering nucleotide excision repair machinery to generate lethal double-strand breaks preferentially in actively transcribed genes. It is approved for unresectable soft tissue sarcoma and relapsed ovarian cancer (in combination with pegylated liposomal doxorubicin), and is under investigation for other malignancies. Its unique mechanism of action in transcription-coupled repair makes it active in tumors with deficient homologous recombination.
