Sulfadoxine is a long-acting sulfonamide antibiotic used in combination with pyrimethamine (Fansidar) for the treatment and prevention of malaria caused by susceptible Plasmodium falciparum strains, particularly for intermittent preventive treatment in pregnancy and in malaria-endemic regions. The combination exploits sequential blockade of the folate synthesis pathway: sulfadoxine inhibits dihydropteroate synthase while pyrimethamine inhibits dihydrofolate reductase, providing synergistic antiparasitic activity. Resistance has limited its use in some geographic areas.
