Technetium sestamibi (99mTc-MIBI, methoxyisobutylisonitrile) is a lipophilic cationic radiopharmaceutical that accumulates in mitochondria-rich cells proportional to membrane potential, used for myocardial perfusion imaging to evaluate coronary artery disease and assess myocardial viability. It is also used for parathyroid adenoma localization and breast cancer imaging (scintimammography) as a supplement to mammography. The agent's retention in well-perfused, metabolically active myocardium allows distinction of ischemic from infarcted tissue.
