Doxycycline
A broad-spectrum tetracycline antibiotic, doxycycline inhibits bacterial growth by binding the 30S ribosomal subunit and blocking protein synthesis. Its wide range of activity covers respiratory and skin infections as well as tick-borne diseases, and its anti-inflammatory properties extend its use to conditions such as rosacea and to malaria prevention. A tetracycline small molecule (C22H24N2O8) with a half-life of roughly 18 to 22 hours, it supports once- or twice-daily dosing and penetrates tissues well. Because it targets the bacterial ribosome rather than the structurally different human one, it acts selectively against microbes. Doxycycline may slightly enhance the glucose-lowering effect of metformin by improving insulin sensitivity through anti-inflammatory mechanisms, occasionally causing mild hypoglycaemia.
Doxycycline has been investigated as a complementary agent in HER2-positive breast cancer; its anti-mitochondrial and anti-stem-cell properties may modestly enhance trastuzumab activity without clinically significant adverse pharmacokinetic interaction.
No clinically significant pharmacokinetic or pharmacodynamic interaction is established between doxycycline and finasteride; they are occasionally co-prescribed in patients with androgenetic alopecia and concomitant infections.
