Adalimumab
A fully human monoclonal antibody, adalimumab binds and neutralizes tumor necrosis factor alpha, a signaling protein that drives inflammation in autoimmune disease. Administered by injection, it treats rheumatoid arthritis, psoriasis, Crohn's disease, ulcerative colitis, and ankylosing spondylitis, among other immune-mediated conditions. Marketed as Humira with numerous biosimilars, it is one of the best-selling biologics. Because it suppresses immune defenses, patients are screened for tuberculosis and hepatitis B before starting and avoid live vaccines during therapy.

Methotrexate is intentionally combined with adalimumab in rheumatoid arthritis and other autoimmune conditions to reduce adalimumab immunogenicity and improve efficacy; co-administration requires monitoring for additive immunosuppression.
Azithromycin may interact modestly with adalimumab-treated patients through immunomodulatory properties, though direct pharmacokinetic interaction is minimal and the combination is used routinely in clinical practice.
Combining adalimumab with cyclosporine produces substantial additive immunosuppression, increasing the risk of serious opportunistic infections, lymphoma, and other immunosuppression-related malignancies.
