Prodrug esterification allows cefpodoxime proxetil to be absorbed from the gut, after which de-esterification in the intestinal wall liberates the active third-generation cephalosporin cefpodoxime. The active compound binds penicillin-binding proteins and blocks the terminal transpeptidation reaction of peptidoglycan assembly; the resulting defects leave the cell wall osmotically unstable and bacteriolysis follows. Its broad spectrum covers common respiratory pathogens, and clinical use spans ear, throat and sinus infections as well as urinary tract infection. Unlike several oral beta-lactams, absorption is enhanced rather than impaired by food. The prodrug is a small molecule of formula C21H27N5O9S2 with a mass near 557.6 g/mol, approved and developed through the final clinical phase.
